The Scan 360 program was created in 2005, with funding from a 5 year grant from the Federal Administration on Developmental Disabilities, and is currently funded with continuation funding from the Central/ West Regional Office of the Massachusetts Department of Developmental Services.
Scan 360 works with culturally and linguistically diverse families in the greater Springfield area who have a family member with any type of developmental disability, assisting them in identifying, applying for and accessing supports and services for their family member. Scan 360 is designed as a short term program, with the goal of connecting families to appropriate services and supports.
After the grant expired, Scan 360 teamed up with New North Family Services (NNFS) to create a One Stop Family Center and Network in Springfield. The aim of this project is to create linkages among a network of human service agencies, service providers and community resources in Springfield in order to better coordinate information and support to families. There is a particular emphasis on outreach to economically disadvantaged, under-served, culturally and linguistically diverse families.
Working with NNFS, Scan 360 has helped many families find the resources they need to live a more comfortable and manageable lifestyle. New North Family Services specializes in family support, service navigation, case management, and other supports to ethnically diverse Springfield area families.
One-Stop Access Services: SC@N 360 and NNFS offer information and referral services, Family Navigation, a resource library, support groups, and trainings based on the needs and interests identified by our families. Trained parent mentors and Family Navigators offer time-limited support to help identify and coordinate resources and assist families to navigate the specialized generic services and entitlements available in Springfield and throughout Massachusetts. Community Outreach: Community is an important part of SC@N 360 and NNFS. We reach out to Springfield families and are visible throughout the neighborhoods at community events, health fairs, and schools. We also maintain a list of support groups available in the community and can direct parents and family members to these groups as appropriate.
